About The Role Location: Hammersmith Day Centre Contract: Fixed term contract until the 31st May 2025 This is a rewarding role within a small, dedicated and experienced team, providing formally arranged support for groups of people living with dementia in a safe and secure environment. We operate from a purpose-built day centre in Hammersmith and provide a minibus transport service. You will: Facilitate the high-quality support and activities we deliver to our clients on a day-to-day basis. Help to transport to clients to and from the day centre. Ensure that Health and Safety requirements are fully adhered to. Maintain accurate client records. About you As part of a small team, you will help deliver a person-centred service for people living with dementia. You will be a team player, have good communication skills and show empathy for the needs of our service users. We particularly welcome applications from people who are representative of the communities we serve in the borough. You are: Experienced in the delivery of high-quality person-centred support in a day care setting. Able to work inclusively with people affected by dementia and their families. Confident with IT.
