Job Title: Social Worker Job Specialty: Social Work - Home Health/Hospice Job Duration: 13 weeks Shift: 12H Days, Monday to Friday, 8 AM to 5 PM Guaranteed Hours: 40 hours per week Experience: Minimum of 1-2 years (no new graduates) License: ND Social Worker License Certifications: BLS, and one of the following: LMSW-AP, MSW, LCSW, CHP-SW Must-Have: Critical thinking skills, flexibility, knowledge of death and dying Job Description: - Provide hospice admission assessments and bereavement assessments - Develop plans of care and follow up on family and client social needs - Assist with the social needs of home health clients as needed - Preferred experience with HomeCare HomeBase EMR system - Work within hospice and home care services, covering assigned areas - Consult with an available SW Manager/Supervisor as needed - Attend a department-specific orientation; expedited based on travel position
