Job Details Grade: 7A Hourly Rate: £17.10 - £18.64 Salary: £31,219.15 - £34,248.64 per annum Contract Duration: Perm Role Purpose We are pleased to be able to offer the opportunities to join the Social Work Assessment Team at Borders General Hospital. We would welcome your application to join us as Health Social Care Co-ordinator. This is a predominantly office based role within Borders General Hospital. We are a multi-disciplinary team with both Social Work and NHS working in partnership with service users, carers, family members and external agencies to facilitate discharges from an acute hospital and Intermediate care settings. You'll work to strict timetables in a fast paced environment providing reports and other data relating to delayed discharges within NHS Borders. Additionally you'll provide administrative and technical support to meet the demand around multi-agency work, discharge management and complex cases. You'll manage a small team providing admin support to the BGH Social Work team.
