SN Practice Facilitator - Band 7



This is an exciting opportunity for a qualified Specialist Community Public Health Nurse (SCPHN) School Nurse or Health Visitor or Community Nurse with Team leading experience , to join Croydon School health Service as Team leader.



We are looking for an experienced SCPHN School Nurse or Health Visitor or Community Nurse with Team Leader experience to work collaboratively with the Operational Lead for School Health and Immunisation Services, and the School Nurse Team . The main role of the School Nurse Team Leader is to ensure leadership and management in the delivery of high quality service and commissioned contract.



Main duties of the job



The emphasis of the role will be working within a skill mix team and working closely with other Clinical Team leaders, Specialists Practice Teachers and Practice educators to develop and monitor clinical standards and outcomes for children and families. You will support staff to achieve their clinical competencies by working and supervising them in clinical practice, demonstrating how they can deliver the healthy child programme and commissioned KPIs, leaders and the school health team in delivery of the healthy Child Programme.



The successful candidate will be part of a school nurse practice facilitators and will be responsible for managing a small Caseload, demonstrate excellence in practice and leadership within the Service.

QUALIFICATIONS TRAINING/ EDUCATION

Essential




  • Registered Nurse

  • NMC registration as Specialist Community Public Health Nurse (School Nurse)

  • Working towards Masters Level education or equivalent experience

  • Working towards practice teaching/ post graduate education in teaching qualification'.



Desirable




  • Leadership/management qualification



RELEVANT KNOWLEDGE & AWARENESS

Essential




  • Knowledge of national and local strategies pertaining to School Nursing including Pan London guidelines, Public Health Agendas, HCP (5-19) and 4, 5, 6 model

  • Demonstrate knowledge of the Children Act, Working Together and London Child Protection Procedures

  • Up to date and comprehensive knowledge on child protection procedures and partnership working

  • Understanding evidence based practice, audit and clinical governance

  • Understanding of professional and personal accountability in line with the standards laid out in the NMC Code

  • Knowledge of equality and diversity policies in the workplace and service provision.

  • Evidence of on-going Continuing Professional Development (CPD) programmes to maintain knowledge and skills for National Standards for specialist practice within public health.



EXPERIENCE

Essential




  • To be able to demonstrate at least 2 years of relevant School Nursing clinical experience, as well as working at a senior level at Band 6 or 7

  • Ability to teach, educate and facilitate in informal and formal situations.

  • Experience of delivering health promotion programmes

  • Ability to provide 1-1 and group supervision

  • Experience of multiagency working

  • Experience of assessing health needs and using them to inform practice in localities.

  • Experience of facilitating group work



SKILLS

Essential




  • Demonstrates excellent clinical skills and can implement these in practice

  • Safe delegation to junior staff and the wider team. Setting standards and competencies and monitoring and recording staff progress.

  • Performance managing staff and challenging unsafe practice of staff not meeting required standards

  • Maintain contemporaneous records according to NMC and organisation's policy.

  • Excellent interpersonal, written and verbal communication skills.

  • Able to provide and receive highly sensitive and contentious information.

  • Able to facilitate reflective practice.

  • Able to prioritise and manage changing priorities

  • Ability to work efficiently as a team member and as a lone practitioner across clinical settings

  • Demonstrate the ability to work in partnership with parents and carers, and other health professionals

  • Computer literate

  • Knowledge of electronic records system
