Job Summary and Qualifications

The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift. In collaboration with other members of the management team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a resource person for staff, and models a commitment to the organization’s vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.

What you will do in this role:

  • Demonstrates clinical competence within the specialty Scope of Practice.
  • Under the direction of Department Director, assists clinical coaches and assures that orientation for employees is completed and in accordance with policy.
  • Pursues own professional growth and development through obtaining/maintaining specialty certification, and seeks opportunities for growth.
  • Possesses strong knowledge base of regulatory standards. Under the direction of Department Director, maintains a state of readiness in accordance with regulations and standards; takes appropriate action to identify and correct gaps.
  • Demonstrates teamwork, flexibility and creative problem-solving skills when dealing with others including those culturally diverse, departmental staff, physicians, patients, or families. Act as charge nurse to support staff and take patient assignment as needed.
  • Works with nursing staff to ensure all nurse sensitive quality metrics (including Core Measures and HCAHPS) are maintained above the National/ NDNQI benchmark through process improvement, reduction in practice variation, and evidence based practice.
  • Works with staff to identify educational and equipment needs, coach and manage performance skills and develop leadership potential.
  • Adheres to hospital and unit policies.
  • Accountable and responsible for required position competencies.
  • Actively engages in Service Excellence initiatives including ICARE and hourly rounding.

  • Demonstrates accountability for essential clinical knowledge to positively affect patient outcomes within the scope of nursing practice while assessing, planning, intervening and evaluating care.
  • Demonstrates strong and accurate clinical judgment and competent or expert nursing skills while following established policies including those related to confidentiality (written, verbal, electronic and digital).
  • Documents effectively, succinctly, and timely according to organizational policy.
  • Incorporates time management and prioritizes for efficiency and productivity.
  • Follows LGMC process for dispensing of medications and narcotic control and employs clinical judgment related to medication administration.
  • Delegates only those tasks for which she or he believes the other health team member has the knowledge and skill to perform, taking into consideration training, cultural competence, experience, and facility/agency policies and procedures.

  • Appropriately teaches patients and families beginning with admission and continuing throughout hospitalization until the last interaction where instructions for home and/or the next provider are accurately provided.
  • Following Synergy Professional Practice Model maintains and grows competency in response to the needs of patients and the organization as demonstrated by activities such as professional organizational membership or certification.
  • Proficient oral and written skills. Collaborates within the Healthcare Environment.

  • Communicates professionally with other team members and colleagues by following approved methods of communication as applicable (I Care, Scripting, SBAR, Huddles, and Bedside Reporting, Leader Rounding)
  • Collaborates with patients, families, and the healthcare team to coordinate care for an effective and current plan of care. Contributes to the Healthcare Environment with Systems’ Thinking
  • Adheres to and promotes the regulations that safeguard the care including but not limited to: Basic Life Support (BLS), National Patient Safety Goals, American Nurses Association Code of Ethics, Standards of Care, Safe Patient Handling, and the Joint Commission Guidelines.
  • Attends staff meetings and engaged in hospital affairs such as Shared Governance as demonstrated through attendance, input, innovative ideas, and volunteering to serve on unit based or hospital councils.
  • Knowledge and application of the cultural foundations including Professional Practice Model.
  • Demonstrates quality systems thinking through gaining and maintaining knowledge of the quality indicators including the nursing sensitive indicators.
  • Promotes a work environment that is free from harassment or lateral violence such as bullying.
  • A professional role model within the community when representing the organization.

What qualifications you will need:

REQUIRED Licensure: One of the following:

  • Registered Nurse License issued by the Department of Health Professions, Commonwealth of Virginia.
  • Registered Nurse with “multi-state privilege" granted to them by virtue of holding a license in one of the states participating in the Nurse Licensure Compact Agreement and who has not established permanent residency in Virginia.
  • BLS, Basic Life Support Certification from the: American Heart Association.
  • PALS, Pediatric Advanced Life Support Certification from the American Heart Association.

Preferred Certifications/Credentials:

  • Member of a national professional nursing organization
  • ACLS certification unless specified as required above
  • Specialty certification from a national accrediting organization

REQUIRED Education:

  • BSN or enrollment in BSN program required within first two years of employment and completion of BSN within five years of employment.

REQUIRED Experience:

  • Two years Registered Nurse experience
  • Demonstrated proficiency in leadership and management within the specialty

PREFERRED Experience:

  • Two years’ experience as RN in a Charge Nurse role within the area of specialty
