Full-time LPN position for our Assisted Living Facility that is the home to 16 residents. Full-time, 80 hours per pay period, 2 pm - 10 pm. Every other weekend/holiday. Full time benefits: IPERS, Health, Dental, Vision, Life Insurance, Long Term Disability. PTO and Sick Leave accrual begins first day of employment. Shift differential for evening/night shift and weekends.

Job Title: LPN
Primary Function and Relationship to Total OrganizationProvides direct nursing care for individual residents with common, well-defined nursing diagnosis. Works under the supervision of an R.N. Organizes those aspects of nursing care for which she/he is responsible for.
Reports to: Director of Nursing
Supervises: Certified Nursing Assistants
Essential Duties and Responsibilities
- Gathers data relative to health status of the resident reports to physician and documents appropriately.
- Renders direct patient care consistent with the level of nursing skills.
- Uses established channels of communication to implement an effective, health care plan.
- Organizes the delivery of standard nursing care in order to use time and resources effectively and efficiently.
- Works with other health care personnel within the organizational structure to manage patient care.
- Participates in the medical plan of care to promote an integrated health care plan.
- Evaluates patient responses to nursing interventions and documents appropriately.
- Maintains accountability for own care and care delegated to others to assure adherence to ethical and legal standards.
- Plans, promotes and participates in patient education and rehabilitation.
- Provides general nursing care to patients. Administers prescribed medication and treatments in accordance with nursing standards.
- Participates in non-direct patient care activities including educational activities, quality improvement nursing committees and other related activities.
- Keeps supervisor informed of changes in patient condition and of care rendered.
- Demonstrates willingness to support staffing and scheduling needs.
- Complies with hospital and nursing service policies.
- Performs functions other than described above due to extenuating circumstances.

Minimum educational requirementsEducation:
A graduate of an approved LPN Program. Must have current license to practice in State of Iowa. Successful completion of BLS within 3 months of employment
Experience:
Nursing Home experience preferred. Following a formal orientation, proficiency in the assigned clinical area is expected within 3 months.

- Shows a positive attitude toward work schedules, department and hospital changes, assignments and other personnel.
