Summary


Levindale is a 330- licensed -bed facility located in Baltimore that offers a full continuum of post-acute services to help patients regain function and vitality after a life-altering illness or injury. Levindale is accredited by the Joint Commission and Commission on Accreditation of Rehabilitation Facilities.



UNIT SUMMARY:



Brain Health Unit (BHU): The Brain Health Unit is part of the specialty hospital at Levindale, it is comprised of two units, and has a total bed capacity of 60. The BHU units are designed to provide a safe environment for older patients with neurocognitive disorders and medical co-morbidities. The nurse will partner with a multidisciplinary team to engage, support, and treat the patient. The length of stay is between 20-25 days, which allows the team the opportunity to assess, implement medication management and a plan of care that is sustainable and that will maximize quality of life. The nurse patient ratio is 1:8-10 for nurses and 1:10 for geriatric nursing assistants. Each unit has a charge nurse that manages a lighter assignment of 4 patients.

JOB SUMMARY:



Provides safe, age-appropriate patient care, and functions as advisor, resource, for patients and their families as a member of the patient care team. Conducts and documents an initial and ongoing assessment and gathers data relevant to the patient's/family individual needs. Develops and documents a plan of care for selected patients/families. Implements, documents and provides care and education based on plan of care. Evaluates and documents the outcomes of nursing practice.





REQUIREMENTS:




  • Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field

  • A minimum of 1 year experience as licensed RN

  • American Heart Association BLS CPR Card

  • Maryland Registered Nurse License

  • Microsoft Office Suite, Basic computer skills, Cerner, Synquest Standard, Office Equipment, Medical terminology

  • Nursing Process skills based on MD Nurse Practice Act, Critical thinking skills, Medical equipment
