Position Summary Reporting to the Manager, Integrated Stroke Unit, you will work under the direction and supervision of a regulated healthcare provider. You will be responsible for providing direct quality personal care to patients in accordance with their respective care plan. You are also responsible and accountable for quality patient care based on the Mission Vision Values and Patient Declaration of Values at BCHS. Primary Responsibilities Practices within an Interprofessional Collaborative Practice (IPC) philosophy and model of care. Demonstrates BCHS policies and procedures to guide their clinical practice. Collaborates with the interprofessional care team in clinical discussions related to observed patient care needs and outcomes. Collaborates with the interprofessional care team to provide relevant patient information when and where required. Collaborates with the interprofessional care team and Registered Nursing staff to ensure the patients established activities of daily living (ADL). Assists in the care process by observing and reporting changes in patients’ physical and emotional condition to the regulated healthcare provider. Assists patients with ADLs such as personal care toileting feeding and recreational activities as noted in the care plan. Assists patients with transfers and ambulation through the use of walkers, canes wheelchairs and other supportive devices as noted in the care plan. Documents patients care needs as per BCHS documentation policy and procedure. Education Requirements Personal Support Worker (PSW) Certificate from a Community College. Current BCLS Certification Mandatory Qualifications Recent hospital experience preferred. Demonstrated ability to function as an effective team member. Excellent interpersonal written and oral communication skills. Demonstrates excellent critical thinking problem solving conflict management and leadership and skills Demonstrated time management and organizational skills. Demonstrated ability in working with a variety of patient populations within a hospital setting. Excellent attendance record Reliable individual with excellent attendance record and able to work all shifts Mandatory Competencies Safety (patient, worker & workplace) is a BCHS Corporate Priority. The successful applicant will demonstrate good stewardship in the identification, reporting and mitigation of unsafe acts or conditions.
