Young Carers Practitioner

We are recruiting for a Young Carers Practitioner to join our Rochdale service, helping young people who have caring responsibilities.

This is a varied and rewarding role which benefits from true Hybrid Working, involving a mix between community working with young people, office working with a great team and home working for added flexibility and work-life balance.

To support our young carers, a Young Carers Practitioner would be expected to carry out the following tasks:

  • Contact new Young Carers and welcome them to our service
  • Gain an understanding of their caring role and develop individual support plans that reduce the impact of their caring responsibilities
  • Make appropriate referrals for a statutory Young Carers Assessment
  • Provide ongoing, time bound, one-to-one support, regularly reviewing their needs and adapting support plans as required
  • Build relationships with and refer Young Carers and their families to broad range of agencies and community assets which meet their needs
  • Support Young Carers to understand the condition of the person they care for and provide information on health and social care services
  • Participate in multi-agency meetings such as team around the family, child protection conferences and child-in-need reviews
  • Develop and deliver community-based peer support opportunities, such as peer support groups, activities and trips, including evening, weekend and out of term time opportunities
  • Develop and deliver training opportunities for Young Carers to help them in their caring role and to provide skills for the future
  • Record and maintain accurate and confidential records
  • Use technology and social media to provide information, advice, and support to young carers
  • Help identify Young Carers through a range of community and engagement events
  • Promote and safeguard the welfare of children and young people and adults

The successful applicant will demonstrate capability to:

  • Work with children and young people, individually and in group settings
  • Use Microsoft Office packages
  • Understand and carry out safeguarding and child protection procedures
  • Complete assessments and support plans
  • Motivate and encourage others
  • Manage a complex and varied workload
  • Work well as a team and individually
  • Commit to anti-discriminatory practice
  • Commit to the values and aims of n-compass
