Salary: £26,250 - £34,650 per annum

The organisation is one of Englands most successful home-grown charities. They have over 1,100 staff and around 100 volunteers providing services for over 20,000 people. Their specialist services include substance use, clinical, employment training and education, housing services, housing support and health, young people, and families services.

The Role

As an Outreach Practitioner you will support people who are homeless or vulnerably housed to increase access to substance misuse support, to sustain accommodation and ultimately move into settled accommodation.

Your duties will include but not limited to:

• You will deliver satellite provisions at co-locations such as GPs, probation, hostels etc.
• You will deliver harm reduction, brief interventions as well as being responsible for the care planning, risk assessing, and case management for service users allocated to you.
• You will have previous voluntary or paid experience of working in a client facing role and using active listening skills on a day-to-day basis.

Skills and Qualifications

• Ideally the successful candidate will have experience within a similar role as well as experience within the sector.
• You must be proactive problem solver, empathetic as well as motivated to do this role as well as being passionate about delivering a high standard of care to some of their most vulnerable service users.
• You must me comfortable with lone working as well as being at ease working in a variety of settings.
