Job Description

We are a dynamic and inclusive Further Education college in Manchester, committed to providing high-quality education and support to all learners. As we continue to grow, we are looking for a dedicated SEND/Alternative Provision Transition Lead to join our team and play a pivotal role in ensuring the smooth transition of students with special educational needs and those from alternative provisions into our college community; and support their journey toward positive destinations beyond college.

You will work closely with students, families, schools, and internal college departments to ensure that each learner's transition is tailored to their specific needs, promoting their successful integration and academic progression, and supporting their pathway to positive destinations.

Key responsibilities will include:

Leading and managing The Manchester College's Transitions Team, ensuring effective collaboration with schools, local authorities, and parents/carers to meet the needs of incoming students with SEND;
Reviewing Education, Health, and Care Plans (EHCPs) to understand the specific needs of each student;
Developing individualised transition plans tailored to each student's needs, strengths, and goals;
Coordinating with students, families, and external agencies to create and implement these plans;
Liaising with feeder schools and alternative provision providers to gather information and insights on incoming students;
Organising transition meetings with key stakeholders, including SENCOs, teachers, and external professionals;
Arranging and conducting college visits, orientation days, and taster sessions to help students familiarise themselves with the college environment;
Collaborating with academic staff and the Supported Learning team to ensure appropriate classroom support and adjustments are in place;
Coordinating multi-agency meetings when necessary to align support strategies;
Maintaining thorough and accurate records to track and monitor applications and transition progress;
Supporting students in planning their next steps and achieving positive destinations beyond college.
