Job Description
Belmont recruitment would like to speak with experienced substance misuse workers with experience working with alcohol clients to work for a private charity along with the NHS within the Surrey area.
Job Summary
This role involves engaging and supporting adult 'High Impact Complex Drinkers' within the Surrey community. The Outreach Practitioner will deliver flexible and adaptable services with no fixed time limitations or thresholds, ensuring accessibility for those most in need. Working collaboratively with a variety of partners, this role focuses on providing comprehensive assessments, ongoing assertive outreach, and tailored interventions to facilitate meaningful engagement and sustained recovery.
Please note, you will be working across 4 locations in Surrey including;
-Guildford
-Redhill
-Walton-on-Thames
-Woking
Key Responsibilities
* Conduct comprehensive assessments, risk evaluations, and develop person-centred care plans.
* Deliver assertive outreach, harm reduction advice, and motivational interviewing to engage service users with complex needs.
* Promote and perform screening (e.g., alcohol screening, BBV awareness) and interventions, including participation in the Needle Syringe Programme.
* Coordinate with partners (e.g., Social Care, A&E, Mental Health, Housing Services) to develop pathways for seamless care.
* Establish and support new outreach locations and networks to enhance service accessibility.
* Facilitate volunteer involvement and service user participation in co-production initiatives, including the WDP Capital Card scheme.
* Actively contribute to multi-disciplinary and partnership meetings to support integrated service delivery.
* Maintain accurate and timely case records in compliance with information governance policies.
Key Requirements
* Experience engaging vulnerable groups, particularly in community or satellite settings.
* Strong understanding of harm reduction approaches for alcohol and other substances.
* Ability to conduct assessments, create care plans, and manage risks effectively.
* Excellent teamwork and interpersonal skills, fostering a collaborative approach with colleagues and partners.
* Knowledge of Safeguarding, GDPR, and relevant Health and Social Care regulatory requirements.
Essentials
-You will need an Enhanced DBS for this role.
-You must have a valid license and access to a car for this role, mileage is reimbursed at £0.45p per mile.
-The role will be paying between £18-£20p/h.
